We accept most PPOs and Medicare.

You may download and print intake forms if you are a new patient or have not been seen in the office for three or more years. Refer to the New Patient forms page.

Be sure to bring your insurance card, a photo I.D, a current list of all your medications as well as supplements, herbs, over-the-counter medications and products used. Be sure to let us know of your allergies and any drug reactions that you may have had in the past.
Payment or copays are due at the time of service. Co-pays are a contractual obligation of your insurance policy and we are required to collect them at the time of service. For your convenience, we accept Visa, MasterCard, Discover and American Express. Full payment for cosmetic procedures is required at the time of service.

Minors must be accompanied by a parent or legal guardian, or have a written release to see the doctor. 

We give reminder calls 48 business hours prior to your appointment. This is to allow adequate time to notify the office a minimum of 24 business hours if you need  to cancel or change your appointment. Changes or cancellations made with less than the 24 hour notice will result in a fee of $50, or $100 for a cosmetic appointment.   

In order to keep our fees as low as possible, it is necessary for our office to be as efficient as possible. Appointments that are changed or cancelled with adequate notice allow the doctor's time and services to be made available for other patients who also have treatment needs. This will benefit everyone by allowing the doctor's time to be utilized as efficiently as possible. Thank you for your consideration and understanding of this policy.
